Enhances Clinical Decision Support

Concurrent Billing with Other Services - RPM and RTM

Paragon's platform enables providers to deliver Principal Care Management services (PCM) along with Remote Patient Monitoring (RPM) at the same time. PCM can be invoiced at the same time as remote patient monitoring as long as the monitored time is not billed twice.

TCM 99495

Within two days of discharge, communication, modest medical decision-making, and a face-to-face visit within 14 calendar days of release are all required.

TCM 99496

Following discharge, 2 days of communication, high-complexity medical decision-making, and face-to-face contact within seven calendar days are indicated.
